NOW ENROLLING!
TO QUALIFY, YOUR CHILD MUST BE 4 YEARS OLD BY SEPTEMBER 1, 2012 and be a GEORGIA resident. (Verified by birth certificate and proof of address).
Please bring the following items to registration:
- Birth Certificate
- Social Security Card
- Immunization Certificate Form 3231
- Eye, Ear and Dental Screen Form 3300
- Proof of residency with Street Address (must be a Power, Water, Cable or Land Line Phone bill).
NOTE: Cell phone bills will not be accepted. - PeachCare, Medicaid, or Food Stamps card (if applicable)
You can obtain forms 3231 and 3300 at the local health department or your child’s doctor’s office.
